Q: Can I get the form myself and send it in to apply?

A:  Since COBRA is a continuation of an employer's group health policy, you can only get the election form through them. The past employer may hire a third party administrator that handles the payments and  sending out any forms or letters.  They are required by COBRA law to mail them to you with-in 45 days of your last day of work.
