Q:I signed up for the HCTC program they want a copy of the paper work that I originally filled out when I signed up for Cobra insurance and also they want to know the end date for my Cobra insurance if you could send me that information that would be great.
A:To get the information from when you were on Cobra, the certificate of coverage, you need to get that from either your Cobra administrator or your past employer.They are who you continued on their group health plan with, therefore they would be able to supply you with that information or put you in contact with who handled it directly and get it to you.
