Q:  We did not receive any information from our employer. We need the appropriate forms to apply.

 

A:  When you lose your job and benefits, if the company has more than 20 employees and there is still a group health plan in force, your employer has 45 days from your last day of benefits to supply you with your Cobra packet; you then have 60 days to elect the coverage.  If you have received that information, you need to speak with your past employer as to why you have not received it.  If they will not supply you that information, and you feel that your rights are not begin met, you can contact the Department of Labor at 866-444-3272.
