My previous employer changed insurance companies, where do I get my new ID card?

Q: My daughter has paid into COBRA since March, 2007.  Her previous employer has changed insurance companies.  She needs to get a new ID card to give to her doctor.  She has not been able to get a response from the new insurance company or the earlier provider to acknowledge her request.

 

A: Your daughter should contact her previous employer.  They should have her new ID cards.  They also need to provide her a summary plan description (SPD).  This is a written document that gives important information about the plan including what benefits are covered.
