Q:  Where do I download an election form for a previous employee?

A:   With in 45 days from your last day the employer is required to mail you the COBRA packet which will include the COBRA election form.  Your previous employer has a 3rd party administrator for processing the COBRA paperwork  The election form you receive from your previous employer may give you the option to sign up on line or by telephone.
