I received a COBRA notice from my employer but it didnt include the election form.

Q:  I got letter from my previous employer and they say I need to fill out and send them the form. But there is no form or payment list. How I can get information about my problem?

A:  You probably received your COBRA Rights information notice. The COBRA packet which includes the election form should be mailed to you shortly. Your employer has up to 45 days to mail it to you from your last day or the last day of group benefits which ever is later. You should contact your employer and let them know you are still waiting for the election form, they will be able to tell you when it will be sent to you.
