Q: Where do I get the COBRA election form to complete. My employer says I need to send in soon.

A: The form should have been given/sent to you by your employer (HR).  Your employer may have hired an outside administrator to administer COBRA benefits for them.  Your employer and or administrator has only 45 days to get you the form.  You have 60 days to elect coverage once you recieve the form.
