Q:  I have a question about my COBRA coverage in the past. I am currently applying for a health plan and it is asking for my start date, termination date, and carrier. I need to know this information.

A:   You will need to contact your previous employer or their COBRA plan administrator.  The plan administrator should have sent to you a "Certificate of Creditable Coverage" when your COBRA ended.  This certificate list the date your COBRA started and ended policy number, and the insurance company (carrier) information.
