Q: I am a former employee of FEDEX as of 08/29/07. I'm interested in continuing coverage, but I don't remember receiving a COBRA package form. I have looked again & again, but I don't have a continuing coverage enrollment form. I was covered under CIGNA through FEDEX. I had medical, dental, & vision coverage for myself & my child. What do I need to do? How much would it cost (out of pocket) for coverage for me & my child?
A: Your employer is legally responsible for sending you the COBRA packet within 45 days from the last day of your group benefits.If your employer did not send you the election form you should contact your previous employer to see if you are still eligible to sign up. If you never received your COBRA packet or if your employer will not give you the election form you should contact the Department of Labor (DOL). You can reach the DOL at 1-866-487-2365. Once you receive the COBRA packet you have 60 days to elect COBRA. If you do not sign up for COBRA during your 60 day election period, you will lose your COBRA rights.
