Q:I need to know my payment history for COBRA coverage between the years of 2003-2004. Also all co-pays for doctor visits and Rx’s. Please help! I need this info. for tax purposes. I am now on social security disability due to kidney disease. I worked for Hewlett Packard for 5 years. If you can provide me with any contact info. I would appreciate it.
A:Your previous employer who offered you COBRA will have records of your payment history or they will give you the contact information for their COBRA plan administrator that will have them.For information on your co-pays and Rx’s you may need to contact the insurance company for that information.Your provider’s office and your pharmacy should also be able to give you a printout of your out of pocket expenses.
